Step 2 - Review the checklist of items you need to be approved.

1.      Verification of Income or verification of amount you are receiving from state disability or SSI.
2.      Rent receipts for five months or rent ledger from landlord.
3.      3 months of bank statements.
4.      Picture ID for all adults in the household
5.      Copy of social  security card for all adults and children in the household
6.      Copy of Lease agreement
7.      3 day pay or quit or letter from landlord with itemized rent due including late fees if any.
8.      Delinquent Utility bills (SDG&E or Water Company) if applying for utility assistance.
9.      Landlord information forms (W-9 form and owner information sheet).
10.   HIV diagnosis verification.
11.  Signed authorization forms  (application packet)
12.  Birth certificates if any children are in the home. (children only)
